What an Alleged $1.8 Billion Fraud Case Reveals About the Need for Quality Addiction Treatment

When someone seeks help for addiction, they should be able to trust that their care puts their health and recovery first.

But a recent federal investigation has raised serious concerns about what can happen if that trust is exploited.

On October 22, 2026, the U.S. Department of Justice announced charges against 15 individuals in several separate health care fraud schemes totaling more than $1.8 billion in fraudulent claims. Among the cases was an alleged $1.6 billion substance use disorder treatment scheme, the largest of its kind ever prosecuted by the department.

The allegations highlight why transparency, accountability, and access to quality addiction treatment are so important.

When Treatment Puts Profits Before Patients

According to federal prosecutors, the scheme involved recruiting people into addiction treatment programs with promises of housing, financial incentives, and other benefits. Some individuals reportedly received little meaningful care, while others were pressured to remain in treatment, so facilities could continue billing their insurance.

No one should have to question whether a treatment provider is prioritizing profits over their recovery.

This case doesn’t reflect the many providers who deliver high-quality, ethical care, and it shouldn’t stop anyone from seeking help. It does show why knowing what to look for matters.

Why Quality Treatment Matters

Addiction is a treatable medical condition, but finding appropriate care isn't always straightforward. Families face confusing insurance information, unfamiliar treatment options, and uncertainty about which programs provide quality care.

Quality treatment should be guided by individual needs, delivered by qualified professionals, and grounded in practices supported by research.

In response to the DOJ findings, Shatterproof CEO Pam Jenkins emphasized:

“Families need reliable information to recognize quality addiction treatment, ask the right questions, and make informed decisions about care. Shatterproof’s Quality Matters Family Guide was created to help families understand what quality care looks like and what to ask treatment providers when seeking help for someone they love."

"But the burden cannot fall on families alone. Strong oversight, transparency, and accountability are essential to protecting people seeking treatment and holding bad actors accountable."

"Quality addiction treatment should be the expectation, not the exception.”

It's worth repeating: this case does not reflect the many providers who deliver high-quality, ethical care, and it should never stop anyone from seeking help. It does show why making quality treatment easier to identify and access matters.
